This site is operated by NLUC PLLC (“we”). It exists to help self-funded employers learn about a direct primary & urgent care benefit and connect with care delivery partners. This page explains what we collect and what we do with it, in plain English.

Information we collect

When you submit the lead form, we collect the details you provide: company name, company website, total employees, your name, work email, and (optionally) phone number.

When you use the coverage search, we collect the location you searched (ZIP, city, or address) and its approximate latitude/longitude so we can show nearby clinics. This data is attached to your lead record if you go on to submit the form.

When you use the site generally, and if you have accepted analytics via our cookie banner, we use Google Analytics 4 to understand aggregate site usage (pages viewed, traffic source, device type). If you decline, no analytics cookies are set. We do not use advertising or cross-site tracking cookies.

How we use it

  • To follow up on your inquiry, prepare a coverage map and savings estimate, and arrange a sales conversation.
  • To share relevant lead context with the care delivery partner operating in your market, so they can serve your group.
  • To understand how the site is used in aggregate and improve it.
  • To prevent abuse (rate limiting, spam detection on the lead form).

We do not sell your personal information, and we do not share it for cross-context behavioral advertising.

Who we share it with

We use a small set of service providers (“sub-processors”) that process information on our behalf:

  • Vercel — website hosting.
  • Supabase — database for lead records.
  • Resend — transactional email (lead notifications).
  • Upstash — rate limiting on form submissions.
  • Mapbox — location search and map rendering.
  • Google Analytics — site usage analytics (only with consent).

We also share lead details with the care delivery partner operating in your market, so they can respond to your inquiry. We may disclose information when required by law, to enforce our rights, or to investigate fraud or abuse.

Cookies

Strictly necessary cookies are set only on the password-protected admin area to keep authorized users signed in. They are never set on marketing pages.

Analytics cookies (Google Analytics 4) are set only after you click “Accept” on the cookie banner. Decline keeps them off. Your choice is remembered in your browser; you can change your mind by clearing site data in your browser settings, which will bring the banner back.

Your choices and rights

You can decline analytics any time via the cookie banner. To request a copy of, correction to, or deletion of any lead data we hold about you, email us at privacy@nextlevelurgentcare.com.

Depending on where you live (including California, Texas, Virginia, Colorado, Connecticut, and Utah), you may have additional rights under state privacy law, including the right to access, correct, delete, or port your personal information, and the right to opt out of certain processing. We honor those rights regardless of state of residence for any lead data we control.

Data retention & security

We retain lead records for as long as needed to follow up on your inquiry and meet our business and legal obligations, then delete or de-identify them. We use industry-standard transport security (HTTPS) and access controls on our admin tools, and our service providers are bound by their own security practices.

Not medical advice

This site describes a healthcare benefit available to self-funded employer groups. It is informational only and is not medical advice. For medical questions, your employees should consult a licensed healthcare provider.

Children

The site is intended for use by employer benefit decision-makers and is not directed to children under 16. We do not knowingly collect personal information from children.

Changes to this policy

We may update this page from time to time. When we do, we will update the “Last updated” date above. Material changes will be noted at the top of the page.
